Unable to Boot After Switching to HEADS

I’ve Qubes 4.2 installation on T480. The root fs is ext2 in a LUKS container. and /boot is ext4. It had Lenovo UEFI and I was using it fine. I flashed HEADS and setup TOTP+HOTP, GPG etc. but I’m unable to boot.

Here is what’s happening,

I successfully see the screen that asks me Disk Password, after I put the password I keep see this line with a timestamp,

[ 74.078144] EXT4-fs error (device dm-0): ext4_check_bdev_write_error:224: comm systemd-journal: Error while async write back metadata

I’m booting with kernel 6.12.42-1.

What I’ve tried so far using Live Fedora 42,

  • I unlocked LUKS and run e2fsck
  • I checked /etc/fstab and grub.cfg for UUID and fstype mismatch(was fine)
  • I recreated the initramfs with dracut using the existing kernel version(qubes).
  • I recreated the GRUB.

Nothing worked. When I chrooted and checked the dmesg I saw “mounting ext2 filesystem using ext4 subsystem” and there is nothing afterwards.
